Integrating Kidney Care: A Nephrology‑Led Model for Coordination, Transplant and Outcomes

Ann Moore, MD, didn’t picture herself in nephrology. In medical school, she thought kidney care wasn’t her calling — until an attending nephrologist changed her perspective.

“I really liked how he interacted with patients, how he knew them,” Dr. Moore recalls. “They knew each other for many years, and I liked that longitudinal experience.”

Building the foundation
Kidney care brings together specialists and resources to deliver truly holistic care—connecting patients to cardiologists, dietitians, social workers, and more. That collaborative model is the backbone of better outcomes.

That connection inspired her career. Today, as DaVita’s medical director in Pasadena, California, Dr. Moore leads a team focused on clinical quality, safety and patient experience. Her leadership philosophy? Collaboration.

“Active leadership is really listening and giving feedback — and being there to advise the team when they need clinical direction. It’s being a part of the team, working together,” she says.

But collaboration doesn’t stop at care delivery in the dialysis center. It opens the doors to life-changing possibilities to help patients understand and explore their treatment options?

Opening doors
Dr. Moore’s team helps patients navigate the complex transplant process. For many, transplant is the best path to longevity and quality of life—but it requires coordination across surgeons, insurance, and education. By guiding patients through every step, the team turns hope into reality.

As kidney care evolves, success is measured through alignment – alignment among providers to help patients manage their care, explore the treatment options suited to their needs, and enjoy a better quality of life marked by better health outcomes.

Redefining care
Value-based care is a model that rewards outcomes, not volume. For Dr. Moore, that means aligning incentives so providers can focus on what matters most: keeping patients healthier, reducing hospitalizations, and improving overall well-being. It’s a shift that transforms healthcare from reactive treatment to proactive health management.

Kidney care isn’t just dialysis. It’s the center of integrated, patient-first healthcare — and collaboration is what makes it possible.
