According to the Centers for Disease Control and Prevention, 60% of adults in the U.S. live with a chronic condition, and 40% of adults live with multiple chronic conditions. Vively will integrate Cerner's industry-leading technology across its home-based offerings, including its electronic health record (EHR), population health management platform and consumer framework, which is designed to help lower the cost of care by supporting care teams to better address the unique needs of MVPs.
